INSTRUCTIONS
FOR USING A PDF FORM ON THE INTERNET
These forms are being made
available as Adobe Acrobat Portable Document Format (PDF) files. You can use them as electronic images to be
able to view and print and then used them in the same manner as any traditional
paper form. Also, these forms are Screen
Fillable Forms (PDF Fillable
Version.) These are similar to the PDF
form images described above except that they may be completed on the screen
before being printed or save to your computer’s hard drive or disk. If you have the Acrobat software configured
as a web browser plug-in application, you can fill-out and print the form
within your web browser. If you are
using the free Acrobat Reader software, however, you cannot save the form with
the fill-in data for use at a later time.
You must have a full version of Adobe Acrobat in order to save the
filled-in data made to this electronic form.
The free reader software will allow you to save the blank form for use
at a later time if you are using the filler software as a web browser plug-in
application.

Right click on
the link below to save the file to disk.

Open the document
using Adobe Acrobat or Acrobat reader.

Use the SAVE AS
command to create a working copy of this form.

Navigate through
the form using the mouse or TAB key. Do not use the ENTER key.

Save a copy on your
computer's hard drive or a disk for your records when the form is complete and/or
click the print bottom of the form to print this form. (You must have a full version of Adobe
Acrobat to save the filled-in data). You
can then submit this printed copy to the appropriate USDA, AMS, office as a traditional paper copy or to submit this form
electronically, go to Step 6.
6 Click on the submit button at the bottom of the form, and the electronic system will send your form to the appropriate USDA, AMS, program office for processing. You will receive an e-mail message from the USDA, AMS, office notifying you of the receipt of your form along with a copy of the form you submitted. Be sure you have entered your correct e-mail address on the form.
